Set in its own unique continuity, the story presents a radical shift in the Spider-Man status quo:
- The Tragedy: The issue opens with a brutal battle against a new, horrific villain named Cadaverous. During the fight, Spider-Man’s arm is badly broken, and Mary Jane Watson is tragically killed.
- A New Hero: Following a 12-year time jump, the focus shifts to Peter and Mary Jane's teenage son, Ben Parker. Ben is struggling with school and a distant relationship with his father, Peter, who has retired from being Spider-Man and lost an arm.
- Discovery: Ben begins to develop spider powers and, with the guidance of Aunt May, discovers his father’s old equipment, setting him on the path to becoming the new Spider-Man.
